Hospice of Chattanooga Registered Nurse (RN) - Case Manager in Chattanooga, Tennessee

Job Summary:

The RN Case Manager is a Registered Nurse managing a case load of patients within their geographical team service area, responsible for the accurate assessment of each patient’s status, problems and needs. The RN in conjunction with other members of the Interdisciplinary Team, designs and implements an individualized Plan of Care from admission to discharge/death. The RN serves as the coordinator of hospice care for the patient and their family. All care directly provided or coordinated by the RN is in conformance with federal/state regulations/Medicare guidelines. We are Joint Accredited. The RN makes visits to the patients, the number per week are based on the acuity of each patient, the visits are documented daily into the electronic medical record system by use of an I-pad in the field.

The normal work week is Monday – Friday 8:00 AM – 4:30 PM, with a rotation of a weekday from 4:30 – 8PM on-call and back-up weekend call to help the weekend nurses if necessary. Associate Experience:

R.N. licensed: 2 years hospital RN licensed: 1 year Licenses or certifications:

Current CPR Certification Registered Nurse (RN) TN R.N. license
