Hospice of Chattanooga Referral Intake Nurse (LPN) in Chattanooga, Tennessee
The Referral Intake Nurse is responsible for receiving and processing referrals for HOC or PCS, preparation of paperwork/file for admission RNs, enters data into system, confirms with billing department, alerts all referral calls/status to Director of Admissions daily, and performs daily audits of new admission charts. Contacts appropriate sources to complete referral process.
MINIMUM QUALIFICATIONS OR EQUIVALENTS:
Graduate of an accredited school of nursing (LPN degree)
Three years experience in the medical field or medical office. Medical coding experience preferred.
Licensure or Certification:
Licensed Practical Nurse in TN
EQUIPMENT USED/SPECIAL SKILLS REQUIRED:
Computer/printer, copier, telephone, and FAX. Proficient in Microsoft Word, Windows, D-base, and Lotus 123. Ability to type 65 wpm. Medical terminology required (i.e., diagnosis and medications). Ability to learn quickly with HMS+ (program unique to Hospice of Chattanooga).
PROCESSES RESPONSIBLE FOR:
INTAKE REFERRALS Answering telephone calls recording detailed and accurate information regarding potential patients to the HOC and PCS programs. Checks fax regularly to not miss any faxed referrals and logs same information as a call. Works cooperatively with other referral intake staff to answer calls timely and ensures department phones are covered adequately at all times, before departing for breaks, meals, and end of shifts. Logs all calls with all required fields and follows up timely and appropriately to all calls. Calls Referring Physician to acquire order for hospice care, if not received. Checks lists daily to ensure no referral goes unattended, followed up and alerts Director of Admissions of any problems, timely follow ups. DATA ENTRY Enters detailed and accurate intake information into the computer system timely. Enters all calls with all required fields accurately and timely into the system. COMMUNICATIONS (s) Keeps the Director of Admissions of all pending referrals and status of referrals. Alerts Director of Admissions of any referral not properly followed up, and/or admitted timely. Communicating all referrals and status of referral through to the Director of Admissions daily. Works cooperatively and closely with the HOC billing office to provide information necessary to ensure billing information is obtained and accurate. Consults Hospice Medical Director, as needed. Directs any and all complaints (internal and external) to Director of Admissions, and in absence, to the Chief Nursing Officer.
